When you reached this website, you will see updated links to the information you may need, including the Worker’s Report of Injury Form. Follow the instructions on this page to fill out the form and have it sent in. The first link is a system Adobe-based document you can fill out online but be sure to print a copy before you enter it into the system.

The other link on this page is a similar pdf document which you can download and edit at home. If you cannot submit the file online, send by postal service with certified mail so that you get a signed receipt when it gets there. The ICA website contains every link to forms you could need during your claim period.

Arizona Worker’s Compensation Laws

This FindLaw website page gives you an at-a-glance overview of what you are entitled to as a worker in Arizona when you are injured on the job. Each statement overview provides a link to a statute of the Arizona Compensation Laws in place.
